Make an Appointment

If this is your first appointment at the Craniofacial Center, please ask your child's pediatrician or specialist to fax or send a written referral to the Pediatric Access Center with the following information:

• Your child's name, your name, address and phone number
• Description of your child's condition
• Insurance information, including an "authorization" for the appointment if your insurance company requires it

After the information is received, a Craniofacial Center representative will call you, typically within two days. Please ask your child's doctor to indicate if your appointment request is urgent.

To contact a Craniofacial Center representative regarding new or follow up appointments, call (415) 476-2271 between the hours of 8 a.m. and 5 p.m., Monday to Friday.  Messages left outside of clinic hours will be returned the following clinic day.
